Outreach is a program that...

identifies seniors who are in need of services and benefits.
advises as to what resources are available and helps clients access them as appropriate.
supports seniors in accessing such resources as appropriate.
recognizes the uniqueness of each individual and their needs.

Why consult the Outreach program?

Many people do not know what services are available in the community; we can suggest those that would be most suitable for you.
We can also provide support before a situation reaches crisis point.
If needed, we can advocate on your behalf with agencies and services.
Our aim, where possible, is to empower clients to be in control of their care.

How does Outreach work?

To address a client’s given requirements, we explain possible service options and, if necessary, look for additional or alternate solutions. It is up to the client to decide what would best serve his or her interests.
Outreach then monitors the client, and keeps in touch with others helping them, such that a support system is in place should the individual’s needs change.

Outreach also...

visits/calls those who are isolated, whether at home or in hospital.
collaborates with other service providers.
matches clients with ‘friendly visitor’ and/or phone-call volunteers.

Outreach works with volunteers by...

interviewing prospective volunteers with a view to client needs.
training them for their selected assignment.
distributing Volunteer Log Report forms and collecting the completed reports on a monthly basis.
maintaining regular contact via telephone.
